Whether you’re looking for housing support, utility assistance, health coverage, senior services, transportation, or more—our Central Intake team will guide you through the process of connecting with the right programs.
Central Intake is a streamlined system that helps individuals and families get qualified for multiple services with just one intake interview. Rather than making multiple calls or filling out different applications for each program, you’ll work with a compassionate intake specialist who helps determine which services you qualify for and walks you through next steps.
Before Your Intake Appointment
What should I expect at my intake appointment?
- Appointments may be required
- Plan to arrive 10–15 minutes early
- Allow 30-45 minutes for your appointment
- Bring documents for all household members
- Services are based on eligibility and funding availability
All information shared is confidential.
What counts as a household?
A household generally includes:
- Everyone living in the home
- Related or unrelated individuals
- Income from all adults may be required
Shared or temporary housing situations can affect eligibility.
How will I be contacted after my appointment?
Agencies typically follow up by phone, email, or mail. Please make sure your contact information is accurate and check messages regularly.
